Applicants should file application by mail between September 4(Monday), 2006, and October 16 (Monday), 2006 |

| Admission Credentials |
| Applicants must hold an undergraduate or graduate degree from an accredited college or university. |
|
| Required Application Materials |
|
Applicants are requested to submit the following documents by October 16, 2006. |
|
- Graduate school application (Form-X1, a portrait photo should be attached)
- Applicant card (Form-X2, a photo should be attached)
- Report of medical check-up (Form-X3)
- Transcript (Applicants who have graduated from graduate school are required to submit both undergraduate and graduate school transcripts.)
- Diploma (Applicants who have graduated from graduate school are required to submit both undergraduate and graduate school diplomas)
- Letter of recommendation (Form-X4, a letter of recommendation written by a professor who will be responsible for your intended program at Juntendo)
- Letter of recommendation (Form-X5, a letter of recommendation written by a department head, a director or someone who currently supervises your work. It should be written in either English or Japanese.)
- Statement of purpose; written in either Japanese (800 - 1,200 letters or English approximately 250 - 750 words)
- Bibliography, if any (Use A4 sheet)
- Reprints of published academic paper(s) (if any)
- Application for credentials test (Form-X6, Applicant who has not taken 18 year school education is required to submit this form.)
- Certificate of research career (Form-X7, Applicant who has not taken 18 year school education is required to submit this form.)
- Language proficiency test scores (A copy of Japanese Government, TOEFL and/or TOEIC, if any)
- Application fee for official approval (20,000 yen)

| Applicants should submit the above documents to the professor who is responsible for your specialty at Juntendo, Hongo 2-1 -1, Bunkyo, Tokyo 113-8421, Japan. |

| Announcement of Result |
|
By judging submitted documents comprehensively, decision of admission for the Graduate School will be made. Successful applicants will be listed on the bulletin board on the first floor of 8th building of Juntendo University School of Medicine, at noon of November 16 (Thursday), 2006. A notification of admission and the official documents for entrance will be later sent by mail to the applicants. |

| Entrance Procedure |
|
The successful applicants must complete the entrance procedures and present the following by December 1 (Friday), 2006. |
|
- A written application form
- Tuition fees (400,000 yen per year Tuition foreign students can be reduced upon request)
- Training and practice fees for laboratory work (150,000 yen per year)
- A written oath

School begins on April 1st, 2007. The admission ceremony is scheduled on 11 AM, April 14 (Saturday), 2007 |
