教室紹介
順天堂大学腎臓内科学講座は、1969年の臓器別講座導入に伴い開設され、長年にわたり腎疾患の診療・研究・教育に取り組んできました。現在は鈴木祐介主任教授のもと、腎炎・ネフローゼ、慢性腎臓病(CKD)、糖尿病関連腎臓病、高血圧、急性腎障害(AKI)、腎代替療法など、腎臓病学の幅広い領域を対象として診療・研究を行っています。
私たちが大切にしているのは、日々の診療から生まれる疑問を研究につなげ、その成果を再び患者さんの診療へ還元することです。腎臓病は、免疫、代謝、循環、加齢、腸内細菌叢など多様な因子が複雑に関与して発症・進展します。当教室では、患者検体を用いた臨床研究と、細胞・動物モデルを用いた基礎研究を組み合わせることで、腎疾患の病態を理解し、新しい診断法・治療法につなげるトランスレーショナルリサーチを推進しています。
国内外の研究機関や順天堂大学内の各研究部門とも連携し、若手医師・大学院生が臨床上の疑問を自ら研究テーマとして発展させ、成果を発信できる研究環境の構築を目指しています。

研究
1.IgA腎症・糸球体疾患
IgA腎症は、当教室が長年にわたり取り組んできた中心的な研究テーマの一つです。
IgA腎症を、腎臓だけではなく粘膜免疫と全身免疫の異常が関与する疾患として捉え、上気道・腸管などの粘膜免疫、自然免疫、B細胞・形質細胞、糖鎖異常IgA、補体などの観点から病態解明を進めています。
特に、粘膜での免疫応答がどのように異常IgAの産生につながり、IgAが腎臓に沈着する機序、どのような炎症や組織障害を引き起こすのかについて研究しています。また、腎生検組織や血液・尿を用いて、疾患活動性や予後を反映するバイオマーカー、新たな治療標的の探索にも取り組んでいます。
IgA腎症以外にも、ネフローゼ症候群、半月体形成性糸球体腎炎など、さまざまな糸球体疾患について、その発症・進展機序や治療反応性に関する研究を行っています。
2.糖尿病関連腎臓病・慢性腎臓病
糖尿病関連腎臓病(DKD)および慢性腎臓病(CKD)の代表的な原因疾患の1つです。当研究では、DKDをはじめとするCKDの発症・進展機序を明らかにし、腎機能低下を予測する新たな方法や治療法の開発につなげる研究を行っています。
腎障害には、糖代謝異常だけでなく、炎症、酸化ストレス、終末糖化産物(AGEs)、腸内細菌叢、栄養、生活環境、心腎連関など、さまざまな因子が複雑に関与しています。これらの因子が腎障害の発症・進展に及ぼす影響を多角的に解析するとともに、血液・尿中の新規バイオマーカーを探索し、CKDの早期診断や腎予後予測への応用を目指しています。
さらに、基礎研究で得られた知見と患者さんの臨床データを組み合わせることで、DKD・CKDの多様な病態を明らかにし、個々の患者さんの病態に応じた、より適切な治療につなげることを目標としています。
3.高血圧・心腎連関
腎臓は血圧調節に重要な役割を担う一方、高血圧そのものが腎障害を進展させます。当教室では、レニン・アンジオテンシン・アルドステロン系(RAAS)をはじめとする血圧調節機構と腎障害との関連について研究しています。
また、心臓と腎臓が相互に影響を及ぼす「心腎連関」に着目し、高血圧、CKD、心血管疾患を一連の病態として捉えた研究を進めています。新たな降圧薬・心不全治療薬が腎機能や蛋白尿、心血管系に与える影響についても検討しています。
4.AKI・尿細管間質障害
急性腎障害(AKI)および尿細管間質障害の発症・進展機序について、炎症、自然免疫、酸化ストレス、虚血、細胞障害などの観点から研究を行っています。
さらに、薬剤や健康食品・サプリメントなどによる腎障害についても、臨床所見と腎病理所見を組み合わせて病態を解析しています。AKIからCKDへの移行機序についても重要な研究課題として取り組んでいます。
5.腎代替療法・透析
血液透析・腹膜透析を含む腎代替療法について、患者さんの生命予後とQOLの向上を目指した臨床研究を行っています。
透析患者における心血管合併症、体液・血圧管理、バスキュラーアクセス、感染症など、日常診療に直結するさまざまな課題を研究対象としています。
腹膜透析については、腹膜機能の維持、カテーテル・出口部管理、感染予防、残存腎機能の保持など、安全で長期的な治療継続につながる研究を進めています。
