Introduction

 Emergency and critical care medicine is “fundamental medicine”. The acute conditions such as trauma, shock and sepsis have been threatening our lives. The approach to solve these problems is essential and challenging.
 Patient's characteristics always mirror social backgrounds. i.e. Japanese society is short of doctors and hospitals to take care of patients with acute conditions. There are more and more elderly patients. The need for emergency and critical care department is increasing. We treat patients with wide variety of acute states from common disease such as flu, headache, fever of unknown origin, and vertigo, to life threatening conditions such as severe trauma, shock, sepsis, burn, poisoning, acute coronary syndrome, and stroke. We take care of patients from primary care in emergency room (ER) to critical care in ICU.
 First, our goal as clinicians is to find the best way for patients and treat them timely, cooperating with the other specialists as needed. Second, as a teaching hospital, we teach all medical students and residents how to deal with patients, find serious conditions, and provide appropriate treatment in ER, and the way to treat critical patients including cardiac life support.
 Finally, we have a wide spectrum of projects for basic and clinical research. The themes are “The pathophysiology under acute stress”, “The mechanism and the finding for treatment in sepsis”, “Brain death and resuscitation”, “Patient safety and clinical ethics”, and “The electric triage system”, etc. We encourage not only postgraduate students but everyone in our department to join international meetings and present their works.
